Information for School Based Therapy
To book a school visit:
- Organise a suitable time with the classroom Teacher via email: firstname.surname@education.wa.edu.au
- See our School Staff information tab for names of Teachers.
- Then complete the School Based Therapy Proforma form - see document at bottom of this page.
- Send completed form as a word document to both the Teacher and school administration: sirdavidbrand.adminteam@education.wa.edu.au
School visit bookings need to be done on a term-by-term basis.
Before completing a school visit, Therapists are required to provide digital copies of the following documentation:
- All external providers in child-related work must hold a valid Working with Children Check
- National Coordinated Criminal History Check Certificate Screen
- Qualification Certificate
These documents are in accordance with the Education Department’s Policies in Public Schools.
